The film set out a vision for what joined-up care might mean for Sam, an elderly widower with multiple long term health conditions.
Sam's Story showed how good integrated care could make a big difference to patients like Sam, and make better use of NHS resources too.
Our new film shows you how that vision is well on the way to becoming a reality for local residents.
'A new chapter, Sam's story in the Wokingham area' sees Sam living locally, supported by integrated health and social care teams working out of three neighbourhood clusters in Wokingham Borough.
The integrated teams work closely together to provide Sam with the joined up care and support he needs 7 days a week.
